International Student Internship Verification Form

Thank you for hosting an intern from Berklee this coming semester. In order to authorize the internship for international students, please complete the information in this form at least five business days prior to the student's intended first day.

The information collected is required to provide authorization for curricular practical training (CPT) to an international student in F-1 status who is registered for an internship course.
