Financial Aid Forms

The vast majority of students selected for verification for the 2024–2025 academic year will have their verification fully processed by Inceptia, but some may receive information requests from Berklee. Click here to learn more.

If documents are requested, you will receive an email to your Berklee email with specific links to the documents needed.

If you receive a missing information request for the Master Promissory Note and/or Entrance Counseling, please go to studentaid.gov to complete both e-documents.

If you wish to decline your federal loans, please submit a loan declination form.
